#2556 Referring and Associated Doctors Names Flipped on Consults

Problem Summary:
The "Associated With" and "Referring" doctors are inverted on the Consult Request printout - the name next to the referring doctor is actually the associated doctor and vice versa. Screenshot attached.

Expected Result:
The referring doctor should be the doctor requesting the consult. The "associated" doctor should be the doctor assigned to the patient in their Master File.

Actual Result:
The referring doctor is the doctor assigned to the patient in their Master File. The "associated" doctor is the doctor requesting the consult.

Steps to Reproduce:
(1) Make sure the patient is assigned to a doctor other than the physician that you're logged in as.
(2) Create a consult
(3) Click "Update Consultation Request & Print Preview"
(4) The "Associated With" and "Referring" doctors will be inverted.

Reproduced In:
12_1 Deb 141 (fresh install) + Ubuntu 12 LTS (fresh install)
