#2505 Consultation Request lacks OHIP # of referrring prac

The OHIP schedule of benefits general preamble specifies the requirements for a consultation request
taken from page GP16 of the April SOB
"b. The request identifies the consultant by name, the referring physician by name and
billing number, and identifies the patient by name and health number."

OSCAR provides all but the billing number. This is at least a theoretical problem for the consultant.

I could fix this easily. The will cause a bit of pain though, as many users have their MSP number in their last name as a hack/workaround for this. So with this fix, they would then see their MSP number twice, which isn't a huge deal. Then they can remove the MSP number from their last name (which also has the advantage that everywhere they see their name, there won't also be an MSP number there too!)
