#2481 Print Preview in the Book Consultation Request, don't displa

Precondition: OSCAR McMaster Test Version 12.x, system and DB up and running.

Steps to reproduce the problem:

1. Launch the browser and open OSCAR McMaster site in the testing environment
2. Log In.
3. Select the Consultation Request Module.
4. From Consultation main page select the patient name.
5. Fill out Consultation Request form.
6. From top navigation bar select “Update Consultation Request & Print Preview” function.

Expected Result:
System displays printable version with personal information including patient sex (F/M) and appointment information. User can is able to print document or exit.

Observed:
Booking Consultation Request printable version don't have patient sex indication (F/M).

The issue with printing patient sex indication has been resolved in both Master and R12_1.
A new bug was introduced [#2700].
