I've seen this particular error when the kernel has _not_ been properly patched for ia64. Install (apt-get) the package 'oprofile-source'. There are detailed instructions for building a kernel module in /usr/share/doc/oprofile-source that may be of help. There's a HOWTO for both ia64 and i386 Debian kernels. Using the Debian 'kernel-package' tools make this pretty straightforward. HTH...
