#40 CA State Form 540 - printing postscript error

When I try to print my CA 540 form, page 1 prints ok, but I get the following error in page 2:

ERROR:
typecheck
OFFENDING COMMAND:
known

After playing with this a bit I found I could print only page 2 and this would work ok.

Then I could also print pages 3-5 and that would work ok.

I've attached a pdf for you to test which fails for me. I removed my private information from it. (Note, the original error would print page 1 ok, then rasterize and print part of page 2 [about half way down the sheet] before printing the next sheet with the error message. When I removed my name, address, SS# and the like, now it won't print any of page 2, unless I specifically print page 2 and then it prints ok. The 2nd sheet printed now has the error message on it.)

It sort of looks to me like a postscript stack error, or some sort of error involving how the postscript is generated. I don't know a lot about the interanals of how PDF prints. I looked for other errors in the printer log, but didn't see any.

I'm running Linux Debian 9.2 Stretch w/ Cinnamon 3.2.7 / Linux kernel: 4.9.0.4-amd64
My printer is a hp CP4020 color laser printer.

I took a look at this troubleshooting tips, but a not sure I really have what it takes to debug this.

    OTS has switched to a new PDF generator that is believed to eliminate these problems.
    It is now used on all the forms. You can edit the produced forms using LibreOffice Draw. Please re-try this with version 15.01 or beyond, and let me know if you see any problems.
