	imm: return ERR_FAILED_OPERATION when ccb oi callback timeout [#57]

	An OI timeout on a ccb callback will always abort the CCB so any reply
	forwarded towards the OM CCB client must be an ERR_FAILED_OPERATION

The ccb upcall must be timeout, so that the ccb get aborted.
