Things to check:

Are you running it on the SoC used by RPi1 and not RPi2?

Is your target powered and using 3.3V Vcc and is your GPIO25 (not pin
25) line connected to target's SWDIO?

Have you tried connecting to the same target while manually pulling
its reset line low?
