The panel that shows list of created forms for an encounter displays new (empty) encounter form. Following 'edit' button brings a new encounter form too. Form_Encounter table has correct data, and correct info is displayed on the list of encounters.
However, your bug report is concerning because this is generally what happens when the ID values in the tables get screwed up (this was a widespread nasty bug several months ago secondary to the new log engine, which perhaps, from your report, was not completely fixed).
Do the form_id numbers in the forms table seem to be incrementing way to quickly?
Do you remember what you did before experiencing this behavior (used a specific patient form etc.)?

You're right, I can't recreate it neither on cvs demo nor on my other instance with complete set of data.
The instance where I experienced this error is light on data - no codes in particular. So, I'd attribute this error to the noise...
I'd not object if you close this bug report.
