Have you modified your OFFL files at all? At line 319, my "teams.php" file has the following:
<input type="hidden" name="mode" value="add">
I doubt this would cause the error you are seeing. Your "teams.php" file must be different than mine. Anyway, for a quick fix, you could try downloading the original OFFL package and replace your current "teams.php" file with the downloaded version...

I had a similar error... my work around while silly works.
Create a new league... login and go to that league.
Then... go to admin and add teams to the league you really want to use... when done use phpmyadmin to remove the league you created, or just leave it there for next year (your pick)

I'm getting the following error when attempting to add user teams to my established fantasy league:
