#4 can't add new categories

the categories are first generated by adding all the
values, including the index, manually.

however, when the next category is added the sequence
generator starts at 1 and overwrites the first category.

either start the sequence generator at a higher number,
or let the system generate the number via it's default
for the table. the latter is better
