#1290 Let extend directive specify cache-timeout and exec-type

Currently an "extend" entry's cache timeout and execution type (exec vs. shell) are configurable only by doing SETs to the nsExtendConfigTable. These changes are volatile (lost on agent restart) and not everybody can count on the ability to perform SETs, so I've added support for a couple of optional flags to the "extend" directive.

I guess this should have been filed under patches. If I have permission to move it there, I don't see how to do so.
