Summary:      new feature: added per-server extraheaders
  the extra headers are merged with the following precedence (the earlier ones have more weight)
  1) application specific headers
  2) per-server extra headers
  3) per-driver extra headers
  This makes it possible to provide in the config files defaults, which can be
  overwritten by certain pages in the applications

- user global network drivers in "nsd-config.tcl"
  This makes it easier to users to extend this file by adding own virtual servers.

- improve behavior and error messages for erroneous configurations.
  Previously, it was quite easy to crash the server without error message (during startup)

- updated sample config files
