[MyAB-users] The following fields were required but not filled in...

I'm trying out MyAddressBook - version 0.1.1, I
believe I have followed all the installation
instructions sucessfully however I can't post data? 

For example when I try to create a new catagory I
enter "Friends" and hit the create button, the
response is 

Error

The 'category name' field must be filled in! 

Viewing my apache error_log I see the following error
:
[Thu Feb 17 12:40:59 2005] [error] PHP Notice: 
Undefined variable:  BODY_ATTRIBS in /var/
www/htdocs/myab/include/top.html on line 28

I'd like to resolve this error so I can tryout
MyAddressBook.  Please let me know if you encoutered
this problem and how you fixed it.
