#28 Implement v-tables for multimethod dispatch

As mentioned in our original OOPSLA 2000 paper, we
could implement multimethod and external generic
function dispatch using dynamically constructed
v-tables instead of the current
chain-of-responsibility. Based on Grothoff's results
with Run-about, this would significantly improve
dispatch performance for deep hierarchies.
