You might want to look at the archives of the SCSI T-10 working group.

You are interested in the tail end of the SCSI SMC draft for the format of the 
Element Address Assignment Page, and the SCSI SPC draft for the format of the 
Mode Sense command in general.

I'm afraid we're at the end of my expertise here. Not having a Sun to play 
with, I am not in a position to try to figure this out myself. Hopefully 
there are other Sun users out there who can help.
