- the video window is treated as a separate application, with it's
own dock icon and own menus. this causes several problems:
-- command-q does not have expected effect (does not quit, just
stops playback)
-- menus and hotkeys for main application are unavailable while
video window has focus
-- video window does not show up in the 'window' menu
-- hiding with command-h or option-click on the minimize button
does not hide all windows

- the video window isn't given focus, and thus is often hidden by
the main window when playback starts

- the video window should have a border. leaving out the border
suggests that it cannot be resized.. which most often isn't the case.

- command-h does not hide the video window. Neither does
command-m (this is shown as the hotkey in the 'window' menu..
hopefully it's a typo, since this is non-standard)
