 cairo については、xft に比べて使いこなせていないため、バグがある可能性は高いです。
 御指摘の問題の原因については、まだ分かっていません。

 ~/.fonts.conf と ~/.mlterm/main の設定はどうなってますでしょうか。
 念のため、mlterm のバージョンも教えてください。
 また、cairo を利用している他のアプリケーションでは、embedded bitmap が使えている
ということですよね(cairo については、よく理解しているわけではないのですが、環境
によっては、cairo 自体が embedded bitmapを使わないようになっているものもあるよう
に認識しています。)。

 なお、次のような設定で、アンチエイリアスしないようにした場合もダメでしょうか。

オプションの--typeに"cairo"を指定すると
TrueTypeフォントの埋め込みビットマップが表示されなくなります。
"xft"を指定した場合は表示されるのですが。
バグでしょうか?

私の環境ではvalgrindで警告が出ていたので以下のパッチを作ってみました。

Fixed a memory corruption.

In x_get_all_config_font_names(), when size == 1 and d_size == 0 and
array[0]->key == DEFAULT_FONT, neither strcpy() nor sprintf() are done, thus
p is left at the first character of the blank buffer font_name_list.
In that case it's wrong to do simply *(p - 1) = '\0'.

同僚からefontというのにunicodeフォントがあるというのをききつけ
インストールしたところきれいな漢字が表示されました。
.mlterm/fontには以下のように設定しました:
ISO10646_UCS4_1_BIWIDTH = -efont-fixed-medium-r-*--%d-*-c-140-iso10646-1
