Removed: debugging log used for previous bugfix.

Fixed: Post-correction Target/Actual ratio was being used to tune
quantisation correction the *wrong* way. This produced a bit-stream
that *never* let the buffer fill. In combination with only partial
pre-fill (why WAS that chosen - VCD standard??) in mplex meany the
(modelled) decoder buffer never actually filledmaking it vulnerable to
under-run with quite tiny variations.

Some changes for the next release.

Fix that makes it possible to compile mplex in mingw32

Trial fix: stop sampling frames at Q floor (where Actual bits *expected* to be
< Target bits) to compute mean Target/Actual ratio after pass2
reencoding.
Tidying: remove some redundant inclusion of config.h

Need to use $CC instead of $_cc in the error message since CC is set at the
top (if not defined as an environment variable).
