from happening on systems which lack fmax().

Oooff... this is more work than I expected. Still only half-way to hooking up a first prelimary look-ahead pass2... sigh...
Original rate-control is preserved and will remain in use for
CBR, stills encoding, and reference/testing purposes.

Put some (initial) documentation in for the -A/--ratecontroller option.

Added pass-1 and pass-2 rate controllers seperate
from the current 'on the fly' rate controller.

Currently, they're just dummies - copies of OnTheFlyRateContoller so I can recycle the useful bits from that but they will rapidly change to suit their intended roles.

Do not fall thru from the 422 case into the 411 code!

Remove extraneous P - it should have been \fB instead of \fPB
