Put SDL_gfx related variables inside an ifdef to avoid 'unused' variable
warnings when building without SDL_gfx

User request: Add -C flag to force CBR

Keeping up with the code changes. The -s (spatial) filter defaults to
disabled now since the -m (3D median) filter does a similar job (as good or
better I'm told).

forgot to remove some comments...

need to include config.h (like all the other source modules do) so that
HAVE_STDINT_H and other defines are in scope.

Fixed a compiler warning where a file was included a 2nd time.

Fixed the problem with the heigth and width. Reported some time back in that tacker item:

Now the vectorscope output really works fast.
Vectorscope and histogram is now faster than realtime, and not flickering any more.

Couple more minor fixes (library instead of libraries when talking about 1
file).

Update the jpeg-mmx notes. It is NOT highly recommended to use jpeg-mmx (in
fact it is discouraged ;)).

Document 0 as disabling processing for -m.
