MPEG-2 encoding has been restored to working order. Still some doubts about
whether its 100% correct again but as far as I can tell any remaining bugs are just sub-optimality rather than crass errors.

mplex now support MPEG-2 mux-ing *and* layer 3 audio.

MPEG-2 encoding with non-progressive DCT in progressive streams and
interlaced streams now fixed and working again. Numerous small code
hygiene improvments. Minor performance increase (no generation of
reference data for B-frames unless statistical logging is turned on.
MPEG-2 multiplexing options in mplex made sane.

Restored at least basic progressive sequence/ frame_dct MPEG-2.
The interlaced stuff is currently horribly broken and requires
attention.

Also a small tweak to mplex to make the audio a little more conservative.
