Complete first cut at splitting into a library with clean hooks
for different frameworks and a YUV4MPEG/mjpegtools command line wrapper.
Also made some progress in tidying up naming of source files and placement
of source in the files to make more sense.

synchronized AltiVec code with changes to mpeg2enc

Encapsulated elementary stream writer.
Remove last global variables (other than fixed look-up tables)

Fixed the -V description

Move encoding control parameters into EncoderParams
structure. Further eliminate reference to global parameters

Encapsulate command-line processing from option setup
and encoder parameter calculation prior to splitting into library
and front-end(s).

Fixed termination of sequence and memory deallocation...

First phase of restructuring to make the encoder re-entrant
and with a properly decoupled I/O components to allow straight-forward
embedding as a library with a gstreamer plugin wrapper.

N.b. the way the quantizer was handled will be representative for
all the SIMD stuff called directly from the main body of the
encoder..

Made now a -R for silence with tthe defualt options, and left the -r for silence with specific options.

With the -r option lav2wav generates now silence if there is no audio in the file. Should work, a little bit inacurrate for NTSC. Also updated the manpage.
