#128 Assessment Bugs - Several

[John Teichman]

Hi Mark,

The person who was creating surveys captured some
issues she was having with assessment as she was
creating the survey templates (she particularly had a
tough time with the Module 10 wrap up). I'll send the
file via email attachment as it is too large to upload.
Thanks.

Please list the several bugs as one bug report for each bug
so we can track them.

Are there work arounds for these bugs? If so we should
probably postpone working on them for a future build.
