#5 Display canned help on startup and on turn switch

Patient responses are mostly limited to yes/no,
numbers, etc.

When it's the patient's turn we could show a list of
sentences in the help window.

I've added a new configuration key
<SourceLanguage>.<Domain>.initialHelpSentences=list, of,
help, items
which is being applied at the doctor's component on startup.

I've preset this in
MedSLT2/gui/cfg/AmericanEnglish.properties for English to:

American English.Pharyngitis.initialHelpSentences=where is
the pain, do you have fever, for how long do you have fever,
is the pain worse in the morning

Feel free to edit the help sentences in the above file to
something better ...
