Fixed Issue 198 and added a test case.

simplify code

cache joint plans in individual plan

the special Plan implementation is package protected, and used only from
JointPlans. It allows to avoid expensive Map lookups, which are
performed only for plans which are not of the specific implementation.

use BinaryMinHeap directly instead of Wrapped

the cost of the map lookup at each add operation made the improvement in
sorting speed useless.

change settings

use nanoseconds

script to analyse the performance of coalition selection

helper method to sample without replacement

remove warning

do not attach dataset anymore
