#16 some usernames are added to table twice

I've noticed that if a username has capital letters, it will
be added to that client's player table twice, once with
the capitalization as the user entered it, and once with
all lower case.

This only occurs on that particular user's client, other
users will see only one version of the name (I've had it
be either the lower case version or the uppercase
version, not sure why).
