After about 20 minutes I encounter severe distortion on the sound coming back from the server, though the sound from me heard by others is apparently still fine. This is cured by clicking on the buffer delay setting (without changing it). I can then go for a similar time before needing to repeat the process. I'm using Jamulus version 3.6.2

I experience similar problems, I always thought it was my connection. It must be the receive buffers overflowing and then the codec can't put the data stream back together. Maybe clicking on the buffer empties it?

This kind of behaviour is very difficult to debug, also because we have no control over the infrastructure except for our home computer and router.
Clicking on the buffer delay setting stops and then starts again the data communication, as does changing any of the communication parameters. Somewhere in the data path between your client at home and the server, wherever it is, there is a communications problem that is resolved by restarting the communication.
About the only things you can do are trying another ISP for your local connection or another server. Either of those suggestions will change the routing on the Internet between you and the server and maybe, hopefully, circumvent the problematic communications node.

One last thing, you can try googling bufferbloat.
A strange term, but there are people trying to optimise the throughput of their routers. Maybe you can find some tips there (if the problem is your router. On can always try)
