Short troubleshooting procedure when the problem occurs:

1. Is irw generating any output?

If yes, it's a client problem. Unlikely in your case.

2. Is mode2 generating any output?

If yes, does it look different from the usual output?
Post some mode2 data and the lircd.conf config file that you are using.

3. Is the receiver still generating interrupts?

You can see the interrupt count doing a "cat /proc/interrupts".
It should increase if there is IR activity.

If no, try if unloading and loading lirc_serial helps.
Try if unplugging and replugging the receiver helps.
