I think I've had similar problems. I too use a serial receiver, and I find that occasionally my serial receiver gets several steps behind. This has happened for a very long time, and has happened using the same receiver with many different versions of lirc and with 2 different computers. Basically, my remote presses get "queued" up. For example, if it's behind 3 presses (most common number I've found) and I hit down, left, power, and then ok, the down, left, and power got queued up. When I hit OK, the down event is handled. Hitting any button then hits left, another button then sends the power, and another button then sends the ok. I bet the issue of "seeing" all the IR traffic may be related, as I've never had the problem occur using my laptop's irda port as a receiver.
