I am having trouble getting the correct rendering for TeX sequences like \=l. The macron appears midway through the 'l'. This occurs even with precomposed characters, such as U+1E38 CAPITAL L MACRON UNDERDOT.

Well, Tex is not able to understand the OpenType-tables. Which glyph would like to combine, is there unicode-slot for it. Have you tried XeTex?
