Maybe it's still using more than 28 kHz output rate, cat /dev/sndstat
will tell. To use higher rates you need to play some tricks with amifb,
this has been discussed on this list.

First of all, you don't ever have to log in as root, nor should you. man
su.

You can have modules loaded at startup by putting their names into
/etc/modules. The more elegant way is to have them be loaded on demand
by defining aliases in /etc/modules.conf (maybe your distro is so old
that it's still called /etc/conf.modules). Again, this has been
discussed here.

Are you using a 4.x X server yet? If not, the Permedia2 chip is probably
hardly used.
