    Because KeePass uses a secure mechanism to view images I can't see how to make preview work.

    Yes, it is possible to view an image when hovering over a link or adjust its full display on the right as a picture preview using HTML and CSS. You can achieve this effect by using CSS for image visibility and positioning.

    KeePass is not a browser, and doesn't interpret any HTML or CSS it encounters. (HTML attachments are rendered by a separate system.) The OP appears to want previews to appear when hovering over links in the KeePass window, which is rendered by custom code, not a web browser. In short, your "suggestion" is impossible to implement.
