#12 Contents broken in v2.0

Contents pane in the chm version of PHP manual doesn't
show all elements of the hierarchy. For example, in the
"Function Reference" section most of the subelements
are shown as files but they are really folders. Version
1.3 of kchmviewer worked correctly.
