You also need to ensure that ivtv.o and test_ioctl are compiled with similar videodev2.h files. Either copy the one from your kernel source tree into both the util and driver directories, and rebuild both, or copy the header file provided in the utils directory into the driver directory, and rebuild the module. This works for me. YMMV of course.
