and if you dont assing those fields ad "mandatory" then it works just like a check box would.
I need to be able to create some checklists in a user template that includes drop down lists which was the whole reason for installing the "Request Templates" for the UI portal in the first place to replace our existing paper-based Access/Change Request Forms. At this present time, we attach these to a User Request or Change Request in iTop. I need to be able to create at least one template (as a start) as noted in the attachment notes. I would appreciate any help as I am not an SQL expert or programmer.

I like the request templates Extension. But what I like to add is the possibility to add checkboxes. Does somebody has an idea to realize this?
Not really, but instead you can use it like this:
