  Log:
  o Add new markup elements to our extension of Docbook.  These are:
  
    <codepoint>, used to markup places where we use the numeric values
      of character set code points in the text.  This element has an
      attribute 'character-set' whole value defaults to 'unicode'.
  
    <grapheme>, for names of the graphemes of a script.
  
  o Add a new parameter entity %character-set.attvals containing allowed
    character set names.
