My bad. I finally got the log verbose level high enough to get some real error messages. I found that I either need to compile from source with Kakadu support or produce tile TIFFs. When I created a tiled TIFF it started working. Now I have to decide which way to go. So far the TIFFs I've created are around double the size of the original JPEG, which is not great.
