Did you put a value in the QLT field?

Also, at the moment, the server cannot handle JPEG source
files, so your 99000.jpg file won't work.

The server should create a log file (on Windows this is
C:\tmp\iipsrv.log by default). Take a look at the end of
this file for more information about why the request failed.
