[Hydrogen-devel] Track output port names

Right now, the names for the individual track output ports are showing 
up as "track_out_[number]_[channel]".  How hard would it be to have the 
ports named for the instrument they output?  For instance, if "Tom Hi" 
is what the name of the instrument within h2, "tomhi_L" and "tomhi_R" 
would be the names of the output ports.  I realize this makes the port 
names dependant on the names of things in the drumkit, but it still 
seems relatively simple.
