Have you tried loading the URL in question in a browser, viewing source, 
and then searching the source for the missing term (and any of its 
displayed variations)? htdig of course indexes what the server returns,
which might differ from the contents of the named file (e.g. due to SSI
or dynamic content).

If the file is not HTML, you would probably need to look at the output
of the associated parser to see what is happening. It is quite possible
that a non-HTML document could contain terms that are not visible when
viewing the document directly.

Is it at all possible that the document was modified between the time it
was indexed and the time you viewed it?

The only other thing that comes to mind is database corruption. Has this
problem persisted across more than one complete rebuild of your search
databases?
