This is fairly scattered but basically what I ended up doing
was following these brilliant 2k3 install instructions
http://www.tjitjing.com/code/article.php?subject=php5_iis6_install
and then giving it another go.

I think the if you're using a binary release of PHP there
are a few problems getting additional modules to work.
