#46 Require approval on new issues

It would be nice to be able to require users to have their
issues approved by a site contact, who can then attach
a budget code if any repairs are necessary.

We would like to have teachers be able to submit issues
but require one of the administrators (principal/assistant
principals) add an appropriate budget code for repairs
and approve it before it appears as an issue to the
technology staff.

Perhaps this could be implemented in the following way:
when a site contact logs in, issues are displayed as
approved or unapproved. They can then go in and
approve them (or not approve them) and add any
necessary details.
