#101 Extension manager

Dear Robert,

The process to create a new extension for Halef currently comprises:

1. Creating a new extension in JVXML in mrcp2-callmanager.xml and associating it with the start URL
2. Restarting Halef
3. Creating a new extension in Asterisk pointing to the JVXML extension
4. Reloading extensions in Asterisk
5. Creting a new extension in JVXML pointing to the Asterisk extension
6. Reloading extensions in Freeswitch
7. Creating a new WebRTC Javascript file pointing to the Freeswitch extension
8. Creating a new HTML stimulus page deploying the new Javascript file

This process is extremely cumbersome, errorprone, and time-consuming. The reality should be as easy as

1. Logging on to the extension manager, creating a new extension and specifying a start URL.

Please include this in the PoC feature list as high priority.
