When running gscan2pdf 1.2.3 from Ubuntu 14.4 I notice that the icons on the toolbar do not scale (double) with the rest of the system on an Ultra-HD monitor.
You are using quite an old version of gscan2pdf, but I don't think that will affect this issue.
The gscan2pdf-specific icons are all svg bar one. These therefore are scaled to whatever is requested. Otherwise stock icons are used, and the size is not hard-coded.
Therefore, I would guess this must be a window manager issue. What are you using? Gnome? KDE?.
I don't have a UHD display, so I cannot test or reproduce the problem anyway.

I still don't think this is a bug in gscan2pdf. I suspect that gscan2pdf is your only gtk2 program, and your gtk2 is not set as you would like. I was able to change the size of the toolbar icons with this:
Hi Jeffrey, thanks for getting back at this. I've tried it out, and this has indeed improved matters a lot.
I was however unimpressed by the OCR results, so somewhere in the last 5 years I've switched to Scanbot (for Android). It can just take a photo of a document.
The results of scanning using a mobile camera look worse (obviously), but the OCR results are often better, and this is more important for my current use case.

If the version is rather old, it's only because the repo gods decided that's the latest version they put in the Ubuntu 14.4 LTS repositories.
I use Cinnamon. The system automatically detects UHD. Everything scales beautifully automatically, except:
