#218 Consolidate identifier lists

Currently there are 3 separate lists of identifiers maintained by different experts:

1. The Code Formatter
2. The Code Proof Reader
3. The Uses Clause Manager

It would be nice to at least optionally have the first two use each other's list or both use the Uses Clause Manager's list.

The Code Formatter now optionally uses the Uses Clause Manager list (revision #5100). Its own list takes precedence.
The same for the Code Proofreader (revision #5102) where the UCM list is only used for determining the case of an identifier, it does not do a fuzzy search. This too is optional.
