You are making it too difficult to reproduce the problem, at least I don't have time to go through a procedure like that. The most common problem is that the map options do not match what the grid in GWC is defined to be, so you get errors returned and they are usually rendered as pink tiles. Here it's not possible to tell whether any tiles are requested at all.
