#110 GEM Light ap, remove text boxes for ‘qualified only’ fields

The element windows for the following fields open with
no text box displayed in the element window. However,
if the user clicks on the first blank line in the dropdown,
a text box appears, allowing the user to enter
unqualified data.

The GEM Light application profile document from 3-17-
03 states that these fields should be qualified only; it
should not be possible for the user to enter unqualified
data in these fields. Can we remove the blank line from
the dropdown and prevent the user from entering
unqualified data?
