#9 When adding an episode of care..make state not show unless..

When adding an episode of care there are checkboxes
for
Related to Automobile
and a empty box for State/Province

The State is the state where the accident happened,
and should not be available until the user checks "Yes"
on "Related to Automobile" accident.

Otherwise the user gets confused as to what belongs in
that box, because right now it is located under "Related
to Other Cause"
