[Frame3dd] CSV Template

Hey, 
I couldn't find a template available for using a .csv instead of a .3dd,
so I adapted the original template. I'm posting this to hopefully stop
others from having to re-invent the wheel. 
It was created with comma separators and " text delimiters.
