Ignore files created by "make coverage_report"

Fix Bug #392; RUNPARSE no longer leaks memory.
The return value from parser() was not being freed.

Fall back to writing to stdout in TraceOutput if there's an error writing to stderr.
On Fedora, writing to stderr fails with EAGAIN from the context of atexit handler.
Falling back to stdout is better than ignoring the output, since that makes the debugheap useless.

Add an empty "default" label to a switch statement to silence a warning about unhandled cases of an enum.

Add some comments to po_helper()

Fix Bug #601; PO now displays lines in procedures defined with DEFINE correctly in the Commander History.

The code intentionally didn't print a newline when the WRITER is [] because, when a procedure
is defined with TO, it prints each line using printfx(), which always writes a newline. However,
for procedures defined with DEFINE, it doesn't use that function, so no newline was written.

The fix is a little bit of refactoring that prints the newline differently depending on whether
the procedures was defined with TO or DEFINE.

Add a "coverage-reports" target, which compiles the code coverage files after a covered executable finishes.
This is mostly to get the (non-obvious) steps required into some kind of script so that I don't have to figure it out again.

Add more tests for APPLY.

Update logic that checks if FMSLogo is running on startup to use wxSingleInstanceChecker on Windows.
Before, it called ::CreateMutex() directly.
Using the wxWidgets class increased the compiled binary size by ~1KB, which is worth the benefit of not having conditional compilation.
There is still Windows-specific logic for naming the mutex and for showing the existing application.

Apply the fix for Bug #598 to NAME.
This fixes a bug where NAME didn't behave correctly if the input variable name is name.variable.input or name.value.input.
As for QUEUE and DEQUEUE, the common case (when the procedure's parameter's don't shadow the variable to be assigned)
is executed within the procedure, not as the macro's output list.
